INVITATION LETTER FOR VISA

COMPULSORY:   In consideration of the criminal responsibilities deriving from art. 12 of Legislative Decree no. 286 of 25 July 1998 and subsequent amendments, by flagging this declaration we undertake to:
1. Provide to EICMA all the information necessary to send to the consular authorities, in particular first name, surname, passport number, date of birth and role in the company of the subjects for whom the preparation of a letter of invitation will be requested, as well as the name of the subject that bears the cost of food and accommodation and, if necessary, the name of the place where the subjects concerned will be staying;
2. Guarantee to EICMA that the information as per point 1 is truthful and complete;
3. Confirm to EICMA that the people whose names are or will be shown for the purposes of the issue of the visas will remain in the places indicated;
4. Guarantee to EICMA having legitimately acquired the personal information of the subjects concerned which we will send you for the purposes of the issue of a business visa;
5. Relieve and hold EICMA harmless of any consequences whatsoever that may derive from the infringement by ourselves, or by the people whose names we will provide, for the purposes of the issue of the visa.

The information provided on this form is required for the processing of the visa application(s) and will be communicated to and processed by the competent authorities of the Schengen Member States for the purpose of deciding on the visa application.
